Jun. 24--Food Establishments With Violations: Red/Blue/Total

Burrito Vaquero Taqueria, Centralia 10/0/10

Food cold-holding equipment (large reach-in) was not cold enough for some (potentially hazardous foods). Please check this out with repair service -- could have circulation problems, also may not be able to cool so much food -- need to evaluate this. Keep all PHF at 41 degrees F or below at all times. (10 Red)

Inspection: June 13

Cascade Peaks Resort Country Store, Randle 10/0/10

Handwashing stations were not in compliance. The on-demand heater for the food prep area station was not functioning to provide warm water. The toilet room for employees is not provided with any sink. Currently using restroom in next building. This is a repeat violation. Provide hot water in food prep area today or close until it is provided. (10 Red)

Inspection: June 15

Quality Inn, Centralia 35/5/40

Person in charge had not been trained in required operating plan and did not know: Procedures for processing waffle batter to ensure it is not held too long at unsafe temperatures, cooling procedures for both hot and room temperature (canned) gravy, hot holding temperature requirements, cold holding temperature requirements, egg cooking procedures (glass bowl in the microwave. The bowl used today appears to be a non-reusable single use bowl from some other food), dish washing procedures. This is a repeat violation. (5 Red)

The person in charge is not the person who washes produce and does not know how they were washed (apples and oranges.) Both hot and room temperature gravy are still beeing colled in covered containers. This is not allowed (and) has been noted many times and must be corrected.

The mini fridge does not maintain safe temperatures and is not allowed to be used to store potentially hazardous foods except yogurt and cream cheese if held in bowls completely immersed in ice. Person in charge had not been told this and yogurts and cream cheese were cold holding between 52.3 to 64.2 degrees F. These were moved back to larger reach-in to cool since they had just been placed in the refrigerator at 6:30 this morning. (10 Red)

The rack for drying cleaned dishes is placed on the grease trap, with degreaser bottles stored immediately next to it. Please move the drying rack up to the drainboard to avoid contamination by chemicals and by dirty dishes, sanitizer bucket (far too strong) above it. (10 Red).

The required operational plan is still not being followed, in addition approval to serve oranges and apples was withdrawn following the last inspection, due to repeated improper washing procedures, however they are still being served. Please discontinue service until written approval is provided from this department. This is a repeat violation. (10 Red)

Waffle batter was cold holding at 72.4 degrees F and must be kept at 41 degrees F or below since it is saved for use the following day. However, it is much easier and strongly recommended to make batter fresh daily and discard leftovers at the end of each breakfast.

Dishes are still not being washed, rinsed and sanitized as required. This is a repeat violation. (5 Blue)

Due to the third violation in two years for improper dish washing, a $100 fine is due to the health department within 10 days.

Inspection: June 12

Tower Street Shell, Centralia 15/0/15

Mountain Fresh deli cooler is still not maintaining safe temperatures and meats and cheeses were cold holding at 49.3 degrees F. All potentially hazardous foods must be kept at 41 degrees F or below at all times. Lunchables were discarded and other items were transferred to another cooler. This is a repeat violation. (10 Red)

There is still no thermometer in the milk section and no digital thermometer to measure temps of PHF when needed. This is a repeat violation. (5 Red)

Due to the third violation in two years for improper cold holding, an operational plan is required detailing how the two violations noted above will be corrected. The plan and associated $250 fee must be submitted within 10 days. Due to the third violation for not providing required thermometers to ensure food safety, a $50 fee is due within 10 days. Total fees are $500. Inspection: June 11

Red violations are those most likely to cause foodborne illness and must be corrected at the time of inspection. Blue violations relate to overall cleanliness and operational conditions and must be corrected by established deadlines or by the next routine inspection.

Any establishment receiving 40 red points or any red point item repeated within an 18 month period is considered a high risk and must be reinspected. An establishment that receives 75 red points or 100 total points (red and blue) on a routine inspection or 40 red points on a repeat inspection will have their food establishment permit suspended.
